The Role – “What we are looking for”

We are looking for a vibrant individual to join our Coordination of Support (CoS) team.

Ideally you will be tertiary qualified within the Community Services field and be passionate about connecting NDIS customers to services and resources. You will be enabling people and/or those closest to them using a strengths-based, person centred approach to people with a broad range of disabilities and complex support needs to connect to services and supports, enabling choice and control for the person.

It will also include developing tailored supports that meet individual needs and allow people to achieve independence in their short and long term goals identified in their NDIS Support Plan.

You will be responsible for:

  • Undertaking the referral and intake process for people provided Improved Life Choices supports as part of their NDIS support plan.
  • Providing support to customers to connect with supports and services matching the person’s goals identified in their NDIS support plan.
  • Help build the confidence and capacity for people to identify, plan and review supports and services independently.
  • Creating sustainable connections and supports with mainstream and disability specific services to support people using requiring specialised support.
  • Experience working with complex support needs
  • The development of innovative service responses, administrative responsibilities and direct service provision.
  • Ensure compliance with all reporting and legislative requirements.

About You

  • Tertiary qualifications and experience in Disability, Social Work, Education, Leisure and Recreation, or related behavioural science.
  • Experience in working with adults and/or children with disabilities and where appropriate their families/carers.
  • Assessment and case work/coordination experience.
  • High level of interpersonal and networking skills.
  • Excellent verbal and written skills
  • Proven ability to achieve outcomes for individuals.
  • Excellent time management skills with the ability to handle multiple conflicting priorities.
  • Strong administration and record keeping skills.

As a Support Worker you will establish appropriate and effective working relationships with our senior customers, carers and families. You will provide quality service by responding to individual needs with flexibility and safety. In line with the customers Individual Care Plan you will provide individual attention, supervision and physical assistance to promote the persons independence, health and well-being while supporting our customers to remain living independently in their own homes.

POSITIONS

We are continuing to grow our Aged Care services in Newcastle and surrounds and as result, we are looking for people to join us as Support Workers.

WHAT’S INVOLVED

  • Personal Care – bathing, showering, personal hygiene and grooming, dressing and undressing, toileting, mobility and transfer.
  • Medication Monitoring – monitor, prompt and supervise a person to take their medication safely and correctly as per a Webster Pak.
  • Domestic Care – vacuuming, mopping, laundry, ironing, cleaning of bathrooms and toilets, shopping and general household tasks.
  • Nutrition – provide assistance to plan meals, preparing meals, re heating of meals, assistance with eating utensils and eating aids, and assistance with actual feeding if required.
  • Social Support – transport to medical and allied health appointments, supporting a person to attend a social activity in the community, or other day-to-day activities.

ROLE REQUIREMENTS

  • Certificate III in Individual Support (Aged Care)/Cert 111 Aged Care or Cert 111 in Home and Community Care
  • Experience working within the Aged Care Sector
  • Current Australian driver’s license
  • Registered vehicle with comprehensive insurance
  • Current First Aid certificate
  • Current Police Check – no older than six months old or willingness to obtain at own expense
  • Flexible to work mornings, afternoons and evenings
  • On call availability and weekend work may also be required
  • Ideally, you be a positive person, who can build a rapport quickly with customers. You will have strong communication skills and the ability to adapt and work with customers from all walks of life.

We require a Volunteer Artist who is willing to spend a couple of hours a week working closely with elderly clients in a residential aged care setting. In return you will be provided an understanding of our programmes goals and objectives, training and continuing support from our Art Programmes Facilitator.

Your typical day will include you setting up the activity, welcoming and including the clients, using materials and equipment supplied to encourage participation and provide support and guidance as requested.

Our outreach programmes aims are to support, maintain, and improve the overall health, physical, emotional and cognitive functioning, and quality of life of our clients through art. We encourage our clients to participate to the extent they wish and in their own way, by exploring and expressing themselves artistically.

Our Ideal Volunteer would have…

  • A Diploma in Fine Art or Diploma in Community Services or working towards such
  • Demonstrated artistic and creative ability
  • Experience working with people of diverse abilities and backgrounds
  • Good interpersonal and communication skills,
  • Ability to be friendly, enthusiastic and positive presence,
  • A sense of empathy and a sense of humour
  • Patience and understanding,
  • Ability to work in a safe and inclusive way
  • A Criminal Record Check or be prepared to undergo one

This position is initially a volunteer position however it could evolve in to a paid position as our programme grows and demand increases

  • CRC no older than 6 months old or have applied for one prior to interview

As an experienced domestic assistant you will provide services to our disabled and elderly clients on a casual basis.

In this position you will deliver house cleaning, laundry and bed linen changing assistance to our clients helping them to remain independent and living in their own homes.

  • Prior experience as a domestic or commercial cleaner
