You have lived independently for most of your life why should that change just because of age? We want you to continue to enjoy life and maintain your connection with family, friends and your local community. At ConnectAbility we value and respect your decision to have control of how you live your life and to stay living in your home as long as possible. ConnectAbility is a local non for profit organisation with over 25 years’ experience providing aged care services to people in the Newcastle, Lake Macquarie and Port Stephens area.

As an Approved Provider we deliver services under the Commonwealth Home Support Program (CHSP) and the Home Care Package Program (HCP) or private paid services if required. Our staff hold a Certificate 111 in Individual support – Aged Care or are trained professional cleaners.


Aged Care Advisory Services

ConnectAbility understands that the Aged Care system can be complex, we advise people on a daily bases on what services are available and how to access them. We support and refer people to My Aged Care to obtain an assessment so they can access services appropriate to their needs.

ConnectAbility offers services that are subsidised by the government so you will never pay the full cost of the service you receive. Each service has a low reasonable fee that you will find fits in your budget. We have a hardship policy to support clients who may be going through difficult times financially.

How ConnectAbility can support you to live your life your way

Commonwealth Home Support Program

The Commonwealth Home Support Program (CHSP) is an entry-level home support service for seniors who need some assistance with daily tasks to live independently at home and in the community. The aim of the CHSP is to help you live as independently as possible, with a focus on working with you rather than doing for you. It is about working on your strengths, capacity and goals to help you remain living independently and safely at home. We understand this can be a difficult step for people to take and will talk with you about what you would like us to assist with.

ConnectAbility can help you with the following CHSP services:

Domestic Assistance: provides basic house cleaning such as mopping, vacuuming, and cleaning of bathrooms and toilets, hanging out washing and changing your bed linen. Staff can also carry out shopping from your supplied list. This assists you to keep your home in the way you choose. We will allocate a trained staff member to visit your home at a time suitable to you.

Social Support Individual: accompanied activities such as taking you shopping or providing transport to doctors’ and allied health appointments and attending social outings.

Social Support Group: we run a Friday – Friendship Club which provides social support, morning tea, lunch, activities, entertainers, guest speakers, bingo and the opportunity to meet new people. The Friendship Club is held at the Jesmond Neighbourhood Centre.

Home Care Packages

The HCP program can help you to live independently in your own home. The program provides a subsidy towards a package of care and services to meet your goals, preferences and needs. A Case Manager will work with you and your family to tailor care and services.

The Home Care Packages Program provides four levels of support:

  • Home Care Level 1 – basic care needs
  • Home Care Level 2 – low level care needs
  • Home Care Level 3 – intermediate care needs
  • Home Care Level 4 – high care needs

Home care packages are not limited to a basic list of services. You can use your home care package funds to purchase a wide range of services including:

Care Services: such as personal care assistance, activities of daily living, nutrition, and management of skin integrity, continence management and mobility.

Support Services: such as domestic assistance, lawn mowing, home maintenance, respite, transport to medical appointments, accessing the community and shopping.

Clinical Services: such as Nursing, Occupational Therapy, Physiotherapy, Speech Therapy and Podiatry.

A Case Manager will support you through the process of understanding what your HCP can provide and walk you through the Home Care Agreement, Individual Budget and Individual Care Plan. Your Case Manager will conduct ongoing monitoring and formal reassessment to ensure that care and services continue to meet your needs.

Friendship Club

The ConnectAbility Friendship Club provides social support through our 2 friendship clubs. These groups enjoy morning tea, nourishing lunches and a variety of activities such as entertainers and guest speakers. We also provide a variety of social outings to various places throughout the Newcastle area.


The Heartmoves exercise program is a low to moderate exercise program designed by the Heart Foundation and the Department of Health. Run by accredited exercise professionals, the classes are suitable for anyone who is not currently exercising or previously exercised. The class is also a great place for people to make new friends and work with others to get moving, and have fun exercising at their own level within a small group. Heartmoves is open to everyone and is designed to be safe for people with stable long term health conditions such as heart disease, diabetes or obesity.

The Heart Foundation’s Heartmoves name lets the public and health professional know that these exercise programs are low to moderate in intensity, have a specially trained and accredited leader, follow safety guidelines and encourage people to start slowly and work safely at their own pace

Mondays and Wednesdays: 2.15pm – 3.00pm
Bookings Required. Cost: $7.00 per class.
Classes held weekly, at The Jesmond Neighbourhood Centre.
