ConnectAbility Australia was founded in 1992 by a group of community members and families to meet the needs of people with significant disabilities.
Since then we have grown and diversified and now provide a broad range of personalised supports for older community members through our Aged Care Senior Solutions team, to children and young people through youth and family counselling and a range of services for people with disabilities. People with disabilities may choose from our community supports, NDIS support coordination, or our accommodation options. ConnectAbility customers reside throughout Newcastle, Lake Macquarie, Port Stephens, Maitland, other Lower Hunter local government areas and on the Central Coast.
Our approach is personal, flexible and centred on the person. Whether you are an older person looking for in home services to maintain independence, are a young person seeking counselling or are a person with a disability looking to achieve goals and dreams ConnectAbility will work with you and, should you choose, those closest to you to ensure you achieve your goals.
Dependent on your requirements, ConnectAbility can assist older people with in-home personal care and domestic assistance, shopping and meal preparation, assistance with medication management, social supports and respite care both in-home and in the community. ConnectAbility will support you to continue to live safely at home maintaining independence in the place you have created memories and feel most comfortable.
We look at creating meaningful relationships and true inclusion for people within the community. Whether you are looking to undertake further education and training, volunteer work or recreational activities using local facilities such as libraries, parks, gyms, shopping centres and other recreational venues. ConnectAbility can work with you to address any barriers that exist for you being able to achieve your goals, dreams and aspirations. We have NDIS Support Coordination experts that can link you with other services and supports within your community, assisting you to take control of your life.
ConnectAbility also have a range of supported independent living (SIL) accommodation options that can assist you to live the independent life you choose in a home that meets your needs whether it is having staff available 24/7 or with occasional supports (concierge model) in an area that meets your needs

Community is very important to the team at Jesmond. Qualified and experienced counsellors operate from Jesmond providing youth and family counselling in a safe space. There are also a number of community education opportunities offered such as language classes, vacation programs for school children, and a twice weekly food bank.
